By answering the The Voices and Faces Project ("Project") survey questionnaire, I agree that the Project and its assignees may reproduce, in whole or in part, my story, (exactly as written by me or as edited or modified by the Project) on the Project's website and in other media published and/or distributed by the Project or its assignees, without territorial, time, use or other limitations, and may use my name and likeness in connection therewith. I specifically acknowledge that all data submitted by me may similarly be used, collated and republished by the Project or its assignees with or without out my name and likeness. I understand that the Project will have the right to use my name, likeness and geographic location to identify me and my story. I further understand that the Project may use my email address solely for the purposes of contacting me regarding my story and for communications about The Voices and Faces Project, including the Project's e-newsletter.
